• Work Site Learning Agreement - The formal contract, signed by all parties. Facilitated by the instructors.

• Work Site Evaluation - While the work site supervisor will be aware of its existence, it is the student's responsibility to deliver a copy of the evaluation in advance of January 26, 2006.
